SMAIC Pre-Admission Form Instructions
1. Form Submission Guidelines:
Complete all required fields on the pre-admission form. Incomplete forms may result in delays or rejection of your application.
Ensure that all information provided is accurate and up-to-date.
Attach all necessary documents (e.g., previous school records, identification documents) as specified in the form.
Submit the form via the online submission portal or in person at the Admissions Office.
2. Required Documents:
- Photocopy of Birth Certificate
- Previous Academic Records (e.g., Report Cards, Transcript of Records)
- Certificate of Good Moral Character from the previous school
- Recent 2X2 Photo (2 copies)
Developmental Assessment (if SPED)
Label each document accordingly.
Compile in a folder and label the folder with the student’s full name.
3. Admission Process:
Once the form is submitted, applicants will receive a confirmation email with further instructions on the next steps of the admission process.
An entrance examination/school assessment may be required. If so, details on the examination date, time, and venue will be provided after the pre-admission form is processed.
Successful applicants will be notified through email or phone within one week after the assessment.
